Dear Dr [GP's name],
I am writing to formally request that my forthcoming first outpatient referral for Oral Surgery Service be made to GREAT WESTERN HOSPITALS NHS FOUNDATION TRUST under my NHS Right to Choose, as set out in the NHS Constitution and the Choice Framework.
The NHS Constitution gives every patient in England the legal right to choose any NHS hospital that provides the treatment they require, for their first outpatient appointment. I have chosen GREAT WESTERN HOSPITALS NHS FOUNDATION TRUST after researching publicly available NHS data on waiting times at HospitalWaits.co.uk, which shows GREAT WESTERN HOSPITALS NHS FOUNDATION TRUST provides Oral Surgery Service services.
I understand that you are required to make this referral on my behalf unless there is a clinical reason to refer me elsewhere. If there is such a clinical reason, I would be grateful if you could explain it during my appointment so we can discuss alternatives.
